If you would like to make a donation to support our efforts, please use the form below. Donations can be made by credit card or check. You can make

A general donation.

A donation to defray specific expenses, like postage for CHD Awareness Week.

A memorial donation to honor the memory of a loved one.

A tribute to honor the accomplishment of a friend or family member.

The Congenital Heart Information Network (C.H.I.N.) is a 501(c)3 tax-exempt organization and 100% of your gift will be used to fund our continuing operations. Donations are tax-deductible as a charitable contribution in the U.S.A. (please consult your tax advisor for details). If you provide an email address, we will send you an acknowledgement email message that you may print as your receipt. If you do not have email, we will mail a receipt.
